CCIH 2025 Award: Transformative Change Agent

The CCIH 2025 Award honors a faith-based organization, team, or project that has achieved significant, innovative, and transformational positive change through their work. The award is tied to the theme for the 2025 CCIH Conference: Change Agents for All.

Faith actors are change agents, including health workers and leaders in any type of health-related work, as well as those preaching from the pulpit, and lay leaders working in communities. They provide care and use their voices for integral mission, demonstrating health equity and justice, resilience, being salt and light locally and globally, bridging gaps and addressing misconceptions, and promoting good health practices and health care. The Transformative Change Agent award recipient will have demonstrated high achievement in enacting positive change in their community or globally.

CCIH Encourages Nominations of Small and Large Programs

This award recognizes an organization that embodies the spirit of ‘Change Agents for All’ by demonstrating remarkable impact, regardless of its size or resources. It celebrates not just the scale of change achieved, but the innovation, commitment, and effectiveness with which an organization has maximized its potential to create positive transformation and demonstrate the love of Christ.

Award Eligibility

The team/project/group should have a recognizable connection to CCIH or a member organization. Their work or project should be current or recent – within the past three years.

Award Selection Process

Award nominees will be selected by the following process:

  • Only CCIH Members, Affiliates, Board, and Staff are able to submit nominations.
  • The deadline for submission will be February 28, 2025.
  • CCIH Staff and the Conference Committee will review nominations and make recommendations to the Board in mid-March.
  • The CCIH Board of Directors will review nominations and select a recipient by late March.

Award Presentation and Announcement

The award will be presented at the CCIH Conference in Washington DC, and announced in CCIH publications and in social media. Live presence at the conference is preferred, or a recorded greeting or live virtual presence is also acceptable.
